Practice Setting
This is a Family Care Partnership Medicare/Medicaid MCO that is operated within an insurance company setting.
Preceptorship Description
Age of patients is 18-end of life. All patients enrolled in Family Care Partnership (FCP) must be financially eligible for WI State Medicaid, and be functionally eligible for the program through the use of the Long Term Care Functional Screen. Therefore, all patients enrolled in FCP are nursing home level of care, and we strive to keep these individuals living in the least restrictive community setting possible, while still receiving the most cost effective care possible. These individuals typically carry multiple comorbidities in the setting of significant socioeconomic barriers, such as poverty, poor health literacy, limited access to food and transportation. Opportunities for clinical learning include advanced physical assessments, care coordination and collaboration with providers, frequent use of complex nursing diagnoses and treatment plans, skill of developing rapport, education of complex illness and comorbidities, coordination of post-hospitalization follow up care. All patients are seen in the community, in their personal residence, which affords a very enlightening opportunity to see "the other side" of healthcare. By experiencing a different viewpoint, it can make you a better practitioner, as you are aware firsthand of the barriers patients experience in the community. I would say that this role philosophically focuses less on the "medical model" of healthcare and more on the "holistic nursing model" of healthcare.
Please NOTE that we currently do not prescribe medications or diagnostics, but these are things that we can direct patients towards with their PCP's. If you are seeking a prescribing/diagnostics experience, this is not it! This is an opportunity to use your "nursing brain" and create high level nursing care plans, provide high level education, and follow through with patients.
