Key Responsibilities
● Provide management and delivery of patient care plans in facilities and communities
● Collaborate with the care team
● Support patients in Northbrook, IL and surrounding areas
Schedule and Shift Details
● Mon-Fri; 8A to 5P. No weekends/holidays/on calls

We are looking for compassionate nurse practitioners with:
- Bachelor of Science (BSN) degree from an accredited School of Nursing
- Master of Science (MSN) degree from an accredited School of Nursing
- A minimum of five (5) years of clinical experience
- Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
- Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred.
Required Qualifications
• Master's degree or higher
• 1+ years of experience with Hospice or Home Health or Palliative Care
• Valid Nurse Practitioner (NP) certification/license in Illinois (IL)
• Valid American Heart Association Basic Life Support (AHA BLS) or American Heart Association Advanced Cardiac Life Support (AHA ACLS) certifications/licenses
• 5+ years of experience with Clinical Experience
